New student registrations can be made online. Follow the links below for instructions and the proper forms. The office is closed through May 2020. During this time you may submit your forms in one of 2 ways:

New student registration begins in January. Call the office for questions or details, (808) 332-6801.
You will still need:
State-issued Birth Certificate (NOT Hospital Certificate)
□ Proof of Residency (e.g. Deed, Rental Agreement, Water, Electric or Gas Bill)
□ Registration Form
□ McKinney-Vento Homeless Assistance Act Form (MVA)
□ Migrant Form
□ Court (Custody) Papers {if applicable}
□ Health Information Questionnaire
□F14 Card (Student Health Record) (Original Cardstock may be obtained from the office)
□ TB Clearance (from Physician or Department of Health)
*Department of Health (DOH), Lihue Office provides FREE TB Clearance on Mondays 1-4PM
(call DOH to confirm time and schedule) or see your Physician
□ Physical Exam (PE) within 1 year of entry date to a Hawaii School

□ All Required Immunizations

Students must attend the school that serves the geographic area where they reside. However, permission
to attend another school may be granted by the Department of Education. Please use FORM CHP13-1 and
submit to your geographic area home school. Visit hawaiipublicschools.org for more information on
Geographic Exceptions.
The child must be 5 years old on or before July 31 to be eligible to attend DOE kindergarten. DOE will not accept a child who is not 5 on or before July 31.
Online Registration is now available for students who were NOT enrolled in a Hawaii Public or Charter school in the last year. You must have all required documents saved electronically and ready to be uploaded:
